Do You Need a Referral for Physical Therapy in Michigan?

Usually not — Michigan direct access explained: the 10-visit/21-day rule, the insurance catch, and how to start this week.

Published Nov 2026 · Reviewed against current Michigan law and payer policy · General information, not medical or legal advice

Short answer: in Michigan you can start physical therapy without a doctor's referral — up to 10 visits or 21 days of treatment, whichever comes first — and prevention or wellness services carry no limit at all. The longer answer involves one catch worth understanding: your insurance plan's rules are separate from state law.

What Michigan law allows

Since 2015, Michigan physical therapists can evaluate and treat without a physician's prescription. Treatment under direct access is limited to 10 visits or 21 days per episode; beyond that, a prescription is required. Services for prevention, fitness, or wellness purposes are exempt from the limit entirely.

The catch: coverage rules are the payer's, not the state's

State law says a PT may treat you; it doesn't force your insurer to pay. Medicare requires your plan of care to be certified by a physician (we obtain that signature — you don't need an appointment first). Some HMO plans require a referral for payment. Many commercial PPO plans require nothing. Auto no-fault doesn't require a prescription, though one strengthens the claim file. The practical answer: call the clinic first — a competent front desk verifies your plan's rule in minutes and obtains whatever paper is needed.

Why direct access matters more than it sounds

Speed. Musculoskeletal problems treated within days recover faster and cost less than those that wait weeks for a physician slot, an imaging order, and a referral. For a locked back or a fresh ankle sprain, starting this week versus three weeks from now is often the whole difference in how long recovery takes.

When we'll send you to a physician first

Direct access makes PTs a front door, and good front doors screen. Red flags — unexplained weight loss, night pain unrelieved by position, progressive weakness, bowel/bladder changes, suspected fracture — get referred out immediately. That screening is part of every first visit here.
